Community Support
INDEPENDENT LIVING PROGRAM
The Independent Living Program provides Activities of Daily Living to people living in their own apartments or homes throughout the Belfast area. These services assist with daily and community living. For instance, people in this program may request assistance with banking, grocery shopping, food preparation, bill paying, medical appointments, understanding documents, and accessing other available resources.
PERSONAL SUPPORTS PROGRAM
The Personal Supports Program provides support services for those people that need assistance gaining skills in the areas of hygiene, bathing, safety in the home, and becoming familiar with the community. People in this program often live with other family members.
VOCATIONAL & WORK PROGRAMS
Vocational and Work Programs are offered in a variety of settings within the Belfast community. These services are offered on a time-limited basis to provide supports to enable people to gain and maintain employment. The program is designed to help people with severe disabilities to obtain and maintain a job within the community that matches their interests, skills, and abilities, while providing individualized training in the areas of work ethics, safety, social and personal skills.
COMMUNITY CASE MANAGEMENT
Building upon the years of experience providing individualized planning Group Home Foundation has become certified by the Department of Behavioral and Developmental Services as a provider of case management services (CCM) for adults with developmental disabilities. These services will be offered in the Northern Mid-Coast area (Knox, Waldo and Hancock counties). Referrals for individuals outside of that area will be considered on a case-by-case basis. The case manager will work with the individual and team members to develop individualized plans that identify needs, resources and linkage to coordinate and monitor them.
